Adventure Camp will be held in West Glacier, Montana at Great Northern Resort.  Transportation from Spokane, WA will be provided for Campers and Volunteers.

Camper Forms & Information

Adventure Camp 101 / Medical - This form is meant to give the reader some background information on Adventure Camp's origins, as well as inform the reader of the medical expectations of those who attend Adventure Camp

Camper Physician Release - You must have this form completed by your camper’s primary care physician or endocrinologist before attending camp.

Camper Medical Check-In Form - Please complete this form and bring it to check-in so that you can provide our medical teams with all the important information regarding your camper’s health.

Camper Expectations - Please review and discuss these expectations with your camper prior to coming to camp.

Camper Supplies to Pack - Please review this list to be sure that you have all the necessities before coming to camp.

Continuous Glucose Monitor (CGM) Policy- If your camper plans to bring a CGM to camp, be sure to review this policy.

Adventure Camp Code - These are the expectations for all campers and volunteers in attendance at camp. Failure to comply with the Adventure Camp Code may result in dismissal from camp.

Dress Code - Be sure to review this dress code when packing.

STIX Diabetes Programs Campership Policy - If you are concerned about your ability to afford camp, please review our campership policy. We will do everything we can to ensure that your camper can attend camp!

Camper Releases - These are the releases for STIX Diabetes Programs that you signed during registration.

Volunteer Forms & Information

Volunteer Physician Release - You must have this form completed by your primary care physician or endocrinologist before volunteering at camp.

Volunteer Code of Ethics - During the volunteer selection and training process, STIX Diabetes Programs will make every effort to provide volunteers with policies that govern our living and working in a healthy and safe environment. This policy must be understood and agreed to by all volunteers and is signed during the application process. Violations of this code may result in dismissal.
